Native IVF is a form of in vitro fertilization performed without hormonal stimulation, meaning the entire treatment takes place naturally during a woman’s menstrual cycle. When is this method used, how does it work, and what affects its success? We explain everything in detail in this article.

What Native IVF Is and When It’s Used

This method of assisted reproduction works with the egg that matures during a woman’s natural cycle. Instead of stimulating the ovaries to produce multiple eggs at once, we monitor only the single egg your body chooses on its own. An IVF specialist tracks the dominant follicle — the one producing the highest levels of estrogen. This follicle is then retrieved in the operating room, and if the embryologist confirms the presence of a healthy egg, it is fertilized in the lab and the resulting embryo is transferred back into the uterus.

Who Native IVF Is Suitable For

This approach may be beneficial for women who:

  • have low ovarian reserve (the number of eggs that can mature and be fertilized),
  • respond poorly to hormonal stimulation,
  • prefer to avoid taking hormonal medication.


Native IVF may not be the first choice for:

  • younger women with good ovarian reserve,
  • couples with no prior IVF experience,
  • situations where multiple embryos are needed, for example for genetic testing.

How Native IVF Works

The principle is similar to conventional IVF, but the treatment is shorter and gentler on the body. Here’s what you can expect under the care of experienced specialists.

Consultation and Treatment Plan

Monitoring the Native Cycle

This is followed by a gynecological exam and ultrasound to track the growth of the dominant follicle, along with blood tests. In some cases, we administer a minimal dose of medication solely to precisely time ovulation — while still avoiding stimulation therapy. Alongside female fertility testing, we also examine male fertility via semen analysis and, if needed, STI screening. Additional steps are then proposed individually by the physician.

Egg and Sperm Retrieval

Once the egg is mature, we perform the retrieval at the clinic under short anesthesia. It’s a quick procedure lasting only a few minutes. If you’re not using frozen or donor sperm, your partner will come for sperm collection.

Fertilization

In the laboratory, the egg is fertilized with partner sperm—most commonly using intracytoplasmic sperm injection (ICSI), one of the most effective lab techniques. The highest-quality sperm is injected directly into the egg. The embryo then develops in laboratory conditions for 3 to 5 days, simulating the environment of the female body.

Embryo Transfer

If the embryo develops properly, it is transferred back into the uterus. The procedure is not physically demanding, as many patients confirm. Fourteen days after the transfer, you can take the first pregnancy test. Regardless of the result, we schedule a follow-up consultation.

How Successful Native IVF Is

In native IVF, one egg equals one attempt. In conventional IVF, the optimal number is around 10 follicles, usually producing about 8 mature eggs, of which approximately 6 fertilize. Outcomes differ among women, but statistically, the chances of pregnancy with native IVF are lower.


Success is influenced by:

  • the woman’s age,
  • egg and sperm quality,
  • hormonal profile,
  • the couple’s overall health.
